The On-Line service is available 24 hours a day. Just select the products you would like and follow the instructions on the screen. Our secure order server will assure maximum security with your credit card orders. All of the data you enter on your checkout page, including credit card information, is encrypted while traveling the Internet so it cannot be intercepted for fraudulent use. An automatic confirmation will be sent back to you shortly. If there are any issues regarding your order, L.A. Promotions will contact you.

Fax Orders:
Please print a copy of your completed order form (do not hit "submit order") and fax to: (502) 244-2524 (Attn: Integrity Healthcare Employee Store Orders). Fax orders are accepted 24 hours a day.

For Shipping information - Please enter your Office address. To qualify for free shipping, all orders will be delivered to Integrity Healthcare Corporate Office in Louisville, Kentucky. If you prefer home delivery, shipping charges will apply.

Unisex - Small (34-36), Medium (38-40), Large (41-43) , X-Large (44-46), XX-Large (48-50)

Ladies - Small (6-8), Medium (10-12), Large (14-16), X-Large (18-20)

Delivery/Turnaround Times:
All orders will be put into production on the 1st and 15th of each month. Orders will ship ~7-10 working days later.

Rush Orders We will try to accommodate all orders whenever possible. Please let L.A. Promotions know your needs.

Back Orders and Cancellations
The Customer Service Department will try to notify you of any backorder situations at the time of the order. If you've ordered more than one item, those in stock will ship.

Return Policies
All sales are considered custom and can not be returned. Any defective merchandise will be exchanged.
